Avoiding fusion in wrist arthritis
نویسنده
چکیده
Advanced non-inflammatory arthritis of the wrist can lead to significant disability. Traditionally, the treatment for arthritis of the wrist which does not respond to either anti-inflammatory medication, splintage, steroid injections and time, would be a total wrist fusion. However, wrist fusion substantially diminishes function1 and is usually avoidable. Two facts should prompt the surgeon to avoid wrist fusion. First, arthritis rarely affects all the joints of the wrist; secondly, there are now several movementsparing procedures which can give predictably good outcomes. The history, examination and investigation of a patient with non-inflammatory arthritis of the wrist, and who is a candidate for surgery, should be directed towards one goal – avoiding total wrist fusion if possible. This article describes the options available for surgical treatment of arthritis of the wrist and emphasises the ways in which the disability of total wrist fusion can be avoided.

Total wrist arthrodesis with wrist fusion rod in patients with rheumatoid arthritis.
OBJECTIVES This study aims to retrospectively review the short-term surgical outcome of wrist fusion using wrist fusion rod (WFR). PATIENTS AND METHODS Six wrists of four female patients (mean age 56 years; range 51 to 62 years) with advanced stage rheumatoid arthritis of Larsen IV or V were performed total wrist fusion using WFR. Clinical outcome was assessed using a numeric rating scale of ...
متن کاملA systematic review of total wrist arthroplasty compared with total wrist arthrodesis for rheumatoid arthritis.
BACKGROUND Management of the end-stage rheumatoid wrist is controversial. The most common treatment is total wrist fusion. Total wrist arthroplasty offers a motion-preserving alternative. Comparison of outcomes for total wrist arthroplasty and total wrist fusion is necessary for evidence-based decision making. The purpose of this systematic review was to scientifically evaluate the existing lit...
متن کاملA Literature Review Comparing the Results of Wrist Arthroplasty and Wrist Fusion in Rheumatoid Patients
The wrist is a complex anatomical structure composed of 3 joints: radiocarpal, midcarpal and distal radioulnar joints these joints provide relatively little bony stability. The stability of the wrist is provided by the soft tissues. Rheumatoid arthritis is a disease of the soft tissues. It affects the synovium and therefore most of these stabilizing structures of the wrist causing pain, deformi...
متن کاملFusion of the wrist in rheumatoid arthritis: a clinical and functional evaluation of two surgical techniques.
We retrospectively compared wrist arthrodesis using the Mannerfelt technique in 19 or an AO-plate in 23 patients with long-standing rheumatoid arthritis. The mean follow-up was for 76 months. Compared with the Mannerfelt fusion group, patients in the AO-plate group reported greater satisfaction with their wrist function (74% vs 37%, p = 0.015). Complications were reported in six wrists in the A...
متن کاملScaphoid excision with four-corner fusion: a biomechanical study.
PURPOSE Partial wrist arthrodesis is a commonly performed procedure for the treatment of posttraumatic wrist arthritis because of its ability to provide pain relief without sacrificing complete wrist motion. The purpose of this study was to evaluate the redistribution of force after four-corner fusion and scaphoid excision, and to correlate the findings with the reported clinical outcomes. ME...
